This patient with diabetic retinopathy and neuropathy lived alone and was socially isolated. He had previously had lesser toe amputations. He presented very late at Casualty. He felt no pain in his foot and could not see the problem. He only sought help because he and his pet dog could smell “something terrible” and the dog was very restless and disturbed, and kept sniffing the foot and barking. Infection was tracking along the sole of the foot. He underwent extensive surgical debridement and drainage.

Note the necrotic areas in the wound bed and the surrounding cellulitis. There was a foul odour that drove the patient to seek help, and the debridings obtained at surgery grew Bacteroides, Pseudomonas aeruginosa and Staphylococcus aureus. Severe infections are frequently polymicrobial. 30 Partial nail avulsion complicated by infection. Note the greyish, macerated wound bed; this may be hard to differentiate from greyness and maceration caused by topical application of phenol to ablate the nail bed.

Even a low-grade infection will delay or prevent healing, and spreading infection will cause wounds to deteriorate with alarming rapidity. The structure and function of the feet, and the fact that they are frequently covered by shoes and, therefore, are not observed closely, can make infection a serious problem. This chapter shows examples of soft-tissue infection, osteomyelitis and fungal infections affecting the feet and legs. Unless mentioned otherwise, these infections have developed in diabetic patients.
